How are bank accounts seized by police? How do they know what bank your money is in?
What is the process they use to find your money and can they freeze the bank account
pending a conviction? Or would they only seize cash?

Bank accounts can be seized with court orders if the funds in that account can with out a shadow of doubt be traced back to have been gained illegally through an investigation. Its usually done in federal cases involving fraud, major drug cases involving money laundering that kinda thing. Local police are pretty limited to what and how they can seize things by state laws. Banks accounts they cant touch.. money found on a person involved in drug sales can be seized along with a vehicle as long as it was used in the commission of a felony same with a house if it was used as a center for felony activity police can seize it but it has to be proved in court before it can be seized..

They cannot seize anyones bank account they are an agency! Only the IRS can do that

I think a judge can issue a warrant to freeze the assets but not take them.
